HAVE YOU MADE ANY CHANGES IN YOUR 
FAMILY STATUS?  ADDRESS?   PHONE?

If you have made any changes in your family status, address or phone # please notify the Office.

  Changes Include the Following:

  A.  Address (Diane).

B.  Phone number (Diane).

C.  Family status:  (Frank).

1.        Married or Divorced.

2.       Children:

A.   New born or adopted.

B.    College:

Leaving home going to college:

 Children are considered a dependent as long as they are taking the proper number of credits in an accredited school to be qualified as a full time student.   Children are a dependent for health insurance up to age 25.

 Have they graduated?

Once they graduate they go off the plan.   However, if you notify Wellmark Adm., your child is entitled to COBRA.

E O B ‘s ((Explanation of Benefits))

Every time a bill is submitted to Wellmark Adm. for a claim you will receive an EOB.   It is your responsibility to check it over and if you don’t understand part of it or think that there is something wrong please get in touch with your Office and ask for our help.

Whenever you go to a NON-Wellmark Administrators Inc. provider (doctor, hospital, clinic, etc.) they can request that you pay them.   They may and they may not send a bill to Wellmark Administrators Inc.   It is your responsibility to see that the bill is sent to Wellmark Administrators Inc.   Any time a NON-Wellmark provider is used, the check is issued to you the participant and it is your responsibility to pay the clinic, doctor, etc.   The check will be sent to you along with the EOB.

Checks issued for prescriptions also come back to you.

PLEASE, take a little time and check over anything you get from Wellmark Administrators Inc.   There could be a check inside, an EOB or they could be asking you for some information in order to process the claim.   If you do not respond when they ask for information they then close the file and the bill is not paid.

Don’t Forget the Sunscreen!

Let sunscreen sink in. If you're like most parents, you probably make it a point to grease your kids up the minute you get to the pool or beach. The mistake here? It actually takes 20 minutes for sun block to take effect — and applying it in the midday glare allows ample time for the sun's rays to penetrate and damage the skin. A smarter approach is to slather sunscreen on your children before leaving the house, says Alexa Boer Kimball, M.D., assistant professor of dermatology at Stanford University Medical Center. "Keep a bottle by the door as a reminder," she says. Then add a new layer of lotion every two hours: Even "all day" protection is diluted by sweat and water.
