Note:
All changes are effective January 1, 2006. There will
be
NO
exceptions made!!
Election (Enrollment) Form
All flex forms must be completed, signed, and
returned to the Brooklyn Office prior to December 31,
2005, in order to participate in the Flex Plan during
2006.
If you do not want to participate in any part
of the Flex Plan, check Item 1, sign and date the
Election Form and return it to the Brooklyn office by
December 31, 2005.
Health Insurance Premiums
Starting January 1, 2006, your share of the health
insurance premium will be paid thru the Flex Plan by
negative election.
You do NOT have to do anything.
If you
do want to have health insurance premiums deducted
after
taxes, you must complete the Election (Enrollment) Form
by filling in the new information and return it to the
office by December 31, 2005.
Employer Dental and Vision Contribution
Starting January 1, 2006, the $12.00 per month Company
contribution to the Flex Plan for the Unreimbursed
Medical Account will also be done by
negative election.
If you do not wish to set aside anything more for
medical expenses, you do NOT need to do anything else.
If you
do want to set aside money for medical expenses
in addition to the company contribution
you must complete the Election (Enrollment) Form by
filling in the additional amount and returning the form
to the office by December 31, 2005.
Dependent Care
Employees who use the Dependent Care portion of the Flex
Plan must fill out a new Election (Enrollment) Form and
return it to the Brooklyn office by December 31, 2005,
in order to participate for the Plan Year of 2006. If
you are using a Dependent Care Contract, this form must
be turned in with your enrollment form.
Spouse’s Insurance
This
negative election
does not
apply to those on their spouse’s insurance. You must
complete both sides of the Flex (Cafeteria) Election
Form, sign, and return it to the Brooklyn Office by
December 31, 2005, to qualify for the plan year of 2006.
NOTE: Contributions made to the Flex Plan for
insurance, medical expenses, or dependent care before
taxes means you do NOT pay Federal Income Tax, State
Income Tax, Social Security, or Medicare Taxes on this
money.
