Health Insurance
« If
at any time you receive a letter from First
Administrators requesting additional information,you
must
respond. If you do not reply to their request, any
pending claims relating to the situation, are closed
until you do.
« Your
health insurance identification card is also a
prescription discount card. The card will help you in
two ways. The first one is you will receive a discount
up front at the time the prescription is filled. You
will be required to pay the entire cost when you pick up
the prescription. Second, the pharmacy will
electronically file your claim with First Administrators
for you. You will receive an EOB from FAI along with a
check to reimburse you for your co-pay, if any.
401K
« If
you have not yet completed the new beneficiary form in
your booklet, please do so and send to the Brooklyn
Office.
« All
changes are done directly with Fidelity either via
telephone or on-line. If you are having any problems,
call Frank Heinen in the Brooklyn Office.
« If
you make changes to your funds and want your current
balance and future contributions to be in the same
funds, you need to make two separate changes. One
change for your current balance and one for your future
contributions. Always watch making fund changes for the
short term trading fees. Fidelity will notify you of
these fees and you will need to approve before any
change will be made.
« Watch
your paycheck to see your year-to-date contribution.
If you decide to raise or lower your contribution,
remember it takes one to two weeks before you will see
the change on your paycheck.
Status Changes
« If
you have a change in address, phone number, or dependent
status, it is very important to notify Human Resources
to keep our files correct for all mail correspondence,
forms processing, etc.
