Manatts is dependent upon their employees for the success of the company. They wish to show their appreciation for that success by extending a comprehensive Wellness Program to all employees. Health care costs, productivity, absenteeism, and work-comp claims are all affected by the wellness of employees.  Simply stated, if we are healthier and more productive we will all share in the success. 

We will be offering the Wellness Program administered by Molloy Wellness Company. Please keep in mind this wellness screening is not a replacement for a physical exam.  All members of the Manatt’s Health Plan are able to obtain one physical exam annually.  We encourage you to take advantage of this benefit.  The health screening offers some basic testing and is designed to help you learn how to be more health conscious with some gentle reminders from the screeners.  Feel free to take any test results you may obtain from wellness to your personal physician for evaluation during your annual physical. Please note the changes made in the incentive program and the fee schedule. 

Medical procedures available in conjunction with the Wellness Program for 2004: 

Health Screening – Includes health risk appraisal, blood pressure, heart rate, lower back flexibility, height, weight, body composition analysis, blood profile (glucose, triglycerides, cholesterol, HDL, LDL, HDL/Cholesterol Ratio) and a private, confidential consultation of your results the same day. You will have the opportunity to participate in the follow-up program to help you with any areas of concern or if you would like additional help with some aspect of your health.  Throughout the year, you will receive educational materials that address a variety of health topics.   

     Employee Cost - Free if un-insured or insured under the Manatt Group Health Plan     

     Spouse Cost - Free if insured under the Manatt Group Health Plan;

     $35.00 if not insured under the Manatt Group Health Plan                                 

 Mammogram –  A mammogram is recommended for women 40 years or older, and those younger with a history of breast cancer in their immediate family. A baseline is recommended at the age of 35 by physicians.  Over the age of 40, a mammogram is recommended annually.  You may go through your own family physician prior to your wellness date or 30 days after the date in which the Health Screening is held at your site. If you are insured under the Manatt Group Health Plan, Manatts will pay for the cost of this test. Please instruct the hospital to bill Manatts at PO Box 535, Brooklyn, IA   52211.  If you are not insured under the Manatt Group Health Plan, you will be responsible for any costs associated with the mammogram. 

Prostate Exam  - If you are going to take advantage of this exam, instruct the Molloy Wellness person drawing blood.  They will do a PSA blood test the day of the screening. You will need to schedule an appointment with your family physician for the exam.  The exam will need to be done either prior to or 30 days after the date in which the Health Screening is held at your site.   If you are insured under the Manatt Group Health Plan, Manatts will pay for the cost of this test and exam. Please inform your doctor doing the exam to bill Manatts at PO Box 535, Brooklyn, IA   52211. If you are not insured under the Manatt Group Health Plan, there will be an additional charge for the blood draw done by Molloy Wellness Company.  This fee will be due at the time of service.
