In accordance with IRS Notice 2015-15 and the Paperwork Burden Reduction Act, Manatt’s Inc. is no longer required to automatically furnish Form 1095-C statements to all full-time employees.
If you were a full-time employee or otherwise enrolled in our employer-sponsored health plan for any month during the 2025 calendar year, you may still request a copy of your Form 1095-C at any time.

A copy of your Form 1095-C will be mailed to you within 30 days of receiving your request.
